Important Notice

Indiana University has enhanced its Employee Self-Service options to include making address and other changes directly through OneStart. The form below that previously handled all such changes should now be used only for Campus Address changes.

These new options, along with the previously released E-Mail and Phone Number functionality provide greater flexibility in notifying the University of personal information changes.

E-Mail Addresses

Preferred and Primary Names (Primary is review only)*
Military Status*
Ethnicity (with the option to specify multiple ethnicities)* - AVAILABLE SOON!

All employees are encouraged to review their personal information to ensure its accuracy and make any changes or updates necessary.
