These awards recognize faculty and staff members for service "above and beyond the call of duty." Service for the benefit of the University as a whole or for any of its units which is non-reimbursed and is not specifically job-related will be recognized.
The awards will be presented at the IUPUI Chancellor's Employee Recognition Convocation, September 23, 2008 from 2:30 to 4:30pm. You are invited to nominate any full-time IUPUI faculty and/or staff member who has worked at the University a minimum of 12 months, using the form below.
DEADLINE for nominations is 5:00p.m. on Monday, July 31, 2008.

In selecting the 2008 recipients of this award, the selection committee will review the following information.
1. Give a brief description of the employee's daily job responsibilities.
2. Please list specific examples of this person's activities that are, in your opinion, above and beyond the responsibilities of his/her daily job. Remember that the Irwin award recognizes service above and beyond daily responsibilities. Employees who perform their daily duties in an exemplary manner will not be considered for this award.
3. State why you feel this employee should receive this award.
4. The following materials are required for each nominee. A. Job description or resume for staff members. B. Curriculum vita for faculty C. Three letters of support.
